Job Description

The post holder will focus on the delivery of specialist pharmaceutical care to oncology, surgical oncology, haematology and palliative care patients on the wards, day unit and the outpatient department of the Hospital and support the other clinical departments and wards where necessary.
Focus on the delivery of specialist pharmaceutical care to oncology, surgical oncology, haematology and palliative care patients on the wards, day unit and the outpatient Provide clinical and professional support and assist in the day-to-day operation of the Aseptic and Dispensary Pharmacy.
Be an active member of the oncology team and clinical pharmacy team.
Support the provision of a hospital-wide medicines management service by covering other wards as required.
Provide out of hours emergency pharmacy services when required

Participate in the provision of ward / clinical pharmacy services, reviewing prescriptions, communicating effectively with appropriate healthcare professionals to solve patient related medicines management problems, minimising clinical risk
Educating patients and carers in all aspects of their drug therapy and medicines management and advising them of any discharge issues.
Records significant information in patients prescription and administration records and medical records to ensure safety in medicines admin istration
Prioritise workload in clinical areas to ensure that the higher priorities are dealt with first
Assist senior pharmacists in the development of clinical pharmacy services and implementation of changes necessary to achieve developments
Develop and utilise specialist knowledge of use of medicines in patients under the care of Consultants within the Oncology Unit Hospital Guidelines and national guidelines
Supervise junior Pharmacy staff (Rotational Pharmacists, pre-registration pharmacists, pharmacy technicians and ATOs) in the provision of ward / clinical pharmacy services, and provide support in day to day clinical pharmacy services
Support the Pharmacy Manager in the provision of specialist knowledge and information for the Cancer Services including providing patients with information, Drug Use Reviews or introduction of new Chemotherapy Regimes.
Undertake audit work under direction of the Pharmacy Manager and liaison with the O ncology Department.
Assist the Pharmacy Manager in the development of clinical guidelines relating to the Oncology Department and the Hospital and their implementation
To attend multidisciplinary team meetings for oncology, haematology and paediatric oncology as agreed by the Pharmacy Manager.
Provide medicines information (to agreed Hospital and national standards) within the scope of the role for the Oncology Service To perform day to day duties for the cytotoxic services including pharmaceutical prescription validation, worksheet and label checks, raw material checks and final product check and release. This may include medicines prepared for clinical trials.
Operate and maintain safe systems of work and work to the standard operating procedures manual.
To answer clinical and technical queries from aseptic unit staff, chemotherapy nurses and doctors on cytotoxic drug dosing, preparation and administration.
To act as the aseptic unit lea d in all aspects of service development and updating of documentation.
To assist in the training of rotational pharmacists, technicians, ATOs and pre-registration trainees.
To ensure that all administrative and managerial responsibilities associated with the provision of the Aseptic Unit are undertaken, including the preparation of duty rotas for staff.

Person Specification

Knowledge / Skills / Qualifications:

Masters Degree in Pharmacy or equivalent
Registration with the General Pharmaceutical Council
Ideally have a Certificate/Diploma in Clinical Pharmacy or equivalent Postgraduate qualification.
IT skills required ability to use email and basic word processing as a minimum
Previous Hospital Pharmacy experience.
Excellent communication.
Handle conflict and other difficult situations effectively
The post holder must be able to work under pressure and meet deadlines and be empathetic and have goo d influencing and persuasion skills
